Overbites and Underbites

For some their lower jaw is forward – think Jay Leno while for others their lower jaw is intruded like they’ve hit the wall.  These conditions can cause havoc in your mouth in relation to your teeth.   However, for most their jaws are in alignment but their teeth may not be.

If the front surface of your upper teeth are more forward than the front surfaces of your lower teeth you have an overbite generally meaning your upper jaw is bigger than your lower jaw.  You may be able to sort of stick your tongue between your upper and lower teeth when your jaws are closed.  In an underbite it’s just the reverse with the lower jaw being bigger than your upper jaw.

Some others have what is called a cross bite like the x crossing of railroad tracks.  These conditions can lead to dental problems since the upper and lower teeth are not coming together correctly.  Think of the gears of a motor not being in alignment.  Over time the teeth of the gears will wear down unevenly and even break off.

You see the same thing on your shoes if your feet are not in alignment the soles of your shoes wear unevenly.  Same with a tire on your car – when out of alignment the ride is less smooth and the tires wear out quicker.

Also when teeth are crooked it is harder to keep them clean leading to gum disease and tooth decay.    Today in dentistry we are able to realign crooked teeth.  It leads to longevity of your dentition and makes it easier for you to keep them clean and decay free.

Genetics and Cavities

Sometimes life can be unfair.  Even if you brush and floss your teeth twice per day every day cavities can still happen.  Don’t be too hard on yourself.  There’s mounting evidence that your genetic makeup may have predisposed you to cavities.  Yes your ancestors are still alive and well and in your neighborhood.

The postulation that your genes are related to your oral health is still a novel concept and more research is needed but commonly cited finding states that genetics may increase your chances of having cavities by 64%.

Of course not brushing and flossing your teeth and eating and drinking foods with high sugar content over time will eventually lead to cavities.  Genes can influence the strength of the enamel of your teeth which is the outer protective coating on your teeth.  For some the enamel is hard and strong but others it is softer and more easily damaged by cavity causing bacteria.

The Case Against Silver- Change Amalgam Fillings for Looks Not Safety

If you have some Silver Dollars in your pocket you are not going to throw them away.  If you were given a sterling silver set for your wedding you’re probably going to keep them.  But, for some if you have silver in your mouth you might be inclined to have them nuggets removed.

The Amalgam Silver controversy is still alive today.  If you are of a certain age you probably have some silver fillings in your mouth.  These restorations can last for decades, some more than fifty years.  Think of something else that a human can create that can last for more than a half century with only little maintenance efforts on your part.  Quite impressive!

Back in the day the choice was gold or silver since white fillings did not exist.  If you were in the limo you were probably getting gold while in the bus or subway silver was for you.  The concern with silver is the filling is an amalgam or a combination of things.  The thing most patients are concerned about is the mercury in the filling.

Mercury is produced by the burning of coal and is a heavy element.  When coal is burned mercury is placed into the atmosphere and since it is a heavy element it falls back down to the surface of the earth.  Most of the planet is water so the mercury enters the oceans where the oysters, lobsters and clams consume it.  The large ocean fish such as tuna, shark and tile concentrate the mercury within them when feeding on the other inhabitants of the seas.  When you eat them you concentrate their mercury content in your body.  With silver filling the mercury is a bound salt so if you swallow your filling the mercury will just pass straight through you instead of accumulating within your body.

Silver fillings are strong but not aesthetic.  Today with the development of white fillings, silver ones are entering the dustbin of history.  With a white filling you can’t see it so it’s like you never had it and you’re even more beautiful.   It took a while for dentist’s to figure out how to successfully place white fillings but today the process is very straightforward.

So do you remove any silver fillings you have?  In my opinion no unless you are not happy with the way they look or if your tooth is becoming grey in color due to leakage of the silver ion into your tooth.  You must realize that every time you perform surgery on your tooth there is a chance of the tooth’s nerve checking out as if saying I’ve had enough.  If this happens root canal therapy can be in your future.

Besides aesthetics one advantage of white fillings is that when they finally fail the filling breaks and generally not the tooth.  When a properly placed silver filling fails decades later generally the filling is still intact but it is the tooth that breaks because ultimately a silver filling is a wedge within the tooth.

Do Your Teeth Need a Facelift?

Is Sticky Candy Bad for Your Teeth?

Everyone likes sweet things but some sweet things have hidden dangers.  Sticky candy such as Gummi Bears, Taffy, Tootsie Rolls and Milk Duds make it difficult to open and close your mouth.  This also can happen when chewing a hard sourdough baguette.

Are All Carbs Created Equal? - How Eating Well Supports Healthy Teeth and Gums

When thinking carbohydrates your thinking bread.  New York City is the bread capital of the world.

We like our pizza, we like our bagels, we like our baguettes and don’t forget croissants.  Of course there’s the liquid carbs as in Beer Here.  Go to Yankee Stadium or Citi Field and your beer is part of the game.  The problem with carbs is that too many carbs means you’re eventually going to get bigger like Clemenza in the first Godfather movie.

As most know; sugar is found in candy, cookies, juices and sodas which can harm your teeth.  But there is also sugar present in all the carbohydrates you eat that can cause cavities.  A handful of pretzels or a piece of rye bread can have the same effect on your teeth as a fudge brownie.

All carbohydrates break down into simple sugars which are converted into plaque which can then cause cavities and gum disease.  Carbohydrates are naturally sticky and adhere firmly to your teeth.  Eating carbs with meals is healthier than eating them as snacks because more saliva is generated at mealtime.  Saliva helps prevent food particles from sticking to your teeth.

If you must drink sodas drink them through a straw limiting their contact on your teeth and preserving your enamel.

Different Type of Dental Specialties

In dentistry there are specialties but all general dentists perform all the procedures of specialists.

Dentistry is somewhat different from medicine.  In medicine Doctors generally deal with one part or treatment of the body.  While in dentistry even though there are specialists most dental treatments are performed by general dentists.  The specialists are for challenging cases.

With proper training general dentists can and do perform treatments of all the specialties.   So what and who are dental specialists.  First they are all general dentists who obtain additional training in one area of dentistry and then limit their practice to only performing specific treatments.

Here they are:  Endodontists who are concerned with root canals

                                Orthodontists who are concerned with movement of teeth

                                Periodontists who are concerned with providing dental treatment to children

                                Periodontists who are concerned with maintaining gum health

                                Oral Surgeons who are concerned with removing teeth

All dentists extract teeth.  This is the final treatment for hopeless teeth.  Today we are able to save teeth which in the past would be extracted.  Difficult extractions, third molar or wisdom tooth extractions and patients who want to be put to sleep for their tooth removal are generally referred to an oral surgeon.

Oral surgeons are also placing implants at this time.

Root canals are mainly done by general dentists.  An advantage to the patient is that the fee charged by a general dentist for a root canal is less than what an endodontist would charge for the same treatment.   Once again it is the degree of difficulty of the tooth involved which determines if the patient should be referred.  In the United States there are not enough endodontists available to perform all the root canals that are needed so general dentists are doing the vast majority of them.

All dentists can and do work on children.  It is a personal decision of the dentist if he or she wants to treat children.  Treating children requires a different mindset and the staff needs to be trained to handle the unique needs of working on adolescents.

Gum surgery and implant placement which is what periodontists perform can and are being performed by general dentists.  It comes down to the training and confidence of the general dentist to perform these treatments.

Finally in orthodontics general dentists are performing a substantial number of treatments.  With the advent of clear aligners such as Invisalign tooth movement is becoming the province of general dentists.  Again it is the parameters and the difficulty of the case that determines if an orthodontic referral is necessary.

In dentistry the general dentist as the name implies is the General.  The specialists are the subcontractors who the general dentist engages if he deems their services are needed.   The average patient generally first goes to their dentist for an evaluation before ever seeing a specialist.

How to Keep Your Smile Looking Young and Healthy

Everyone wants to be Healthy and Healthy Teeth Are Part of the Equation

Most of you have teeth.  Teeth are good especially if you like food and food is necessary if you want to stay alive.  There will come a time in life when eating will be your greatest pleasure.  So you want your teeth to last and to last you need to make a little effort to help them survive.

Your teeth want to be cleaned on a daily basis.  Think of each tooth as a little square box.   There is a top front, back and sides.  The bottom or basement is in your jaw so this part may not seem apparent.  Tooth brushing gets the top, front and back.

All the bacteria in your mouth are anaerobic, meaning without oxygen.  When you brush your teeth you bring air into your mouth which contains 20% oxygen.  The oxygen destroys the bacteria helping you to prevent dental decay. 

However, your toothbrush cannot get to the sides of most of your teeth unless you have a missing one.  These areas also need to be cleaned so you need to floss.  You know that thin little string that can get in between your teeth.  Just like the toothbrush the floss pulls air between your teeth and the oxygen within helps to destroy the bacterial colonies always present in your mouth.

Bacteria like warm, dark places with food.  So to keep your mouth and teeth healthy you need to floss and brush in the morning and at night.  Before you go to sleep by cleaning your mouth you decrease the bacteria present and upon waking you remove the buildup that has occurred while you’re asleep. Everyone knows the experience of morning mouth.

What you consume also plays a part in keeping your mouth healthy.  The outside of your teeth are made of enamel, the strongest substance in your body but even this material can become damaged by eating too much sugar or acidic foods such as citrus fruits.  Calcium rich foods such as cheese, milk and nuts protect tooth enamel.

Also, drink plenty of water to keep your body hydrated. Water also serves to wash away foods recently eaten within your mouth.  If you have dry mouth due to aging and/or medications you are on, chewing gums stimulates saliva keeping your mouth moist which is a good thing.

Do Veneers Make Your Teeth Look Bigger?

As an alternative porcelain veneers can be done.  These are thin pieces of porcelain that are bonded to the front of your anterior teeth to correct the unacceptable color or shape of these teeth.  For veneers to look natural we need to reduce some of your teeth in most cases.  If no reduction is done the finished veneers can look too big and bulky.

Why it’s Critical to Replace Missing Teeth

When you lose a tooth, do you miss it?  For some they do while for others they do not.  Hopefully you are attached to your teeth and they remain attached to you.  But, sometimes things just happen.

You’re in an accident, a large filling fractures and then fractures your tooth, you bite down hard on a piece of bone or an olive or cherry pit and suddenly a tooth departs.  If it is a front tooth you will need to act.  Most people do not want to be a gap tooth person when they smile.  If it’s a back tooth some may ponder do I really need to replace it?

The first and most paramount question is do you miss that tooth?  If so, there are several ways to replace it.  If you don’t miss it here are some of the consequences if a tooth is not replaced.  Think of your teeth as the pylons supporting a pier going out into the ocean.  If some of the pylons collapse eventually the pier will also collapse.

Your back teeth have three roots, your side teeth have two roots and your front teeth have one root.  When you lose a back tooth you lose three roots or three pylons.  However, you lose more.  Without the tooth the opposing tooth is no longer in function.  So instead of three roots you have lost six roots.  One tooth loss is not going to kill your bite but as you lose or remove more teeth the remaining teeth are not strong enough to hold your bite. 

Also when you lose a tooth the supporting bone starts to disappear without the tooth present.  The bone is there when the tooth is.  No tooth leads eventually to no bone giving you that gaunt look.  Might be good for trick or treating on Halloween but probably not anywhere else.

Nobody loses all their teeth at once unless in a terrible accident.  It’s a slow progression like the domino effect.  In addition to losing support when you lose a tooth, the tooth behind it now has the ability to move forward.  Unfortunately, the tooth does not move horizontally but generally begins to tip, like the leaning Tower of Pisa.

When you have fewer teeth in your mouth the remaining teeth are under more pressure causing dental restorations in these teeth to have a better chance of fracturing and needing to be replaced.  Today we have the ability to replace a missing tooth without involving the adjacent teeth.  When replaced the new tooth will look and feel like your original tooth.

The Wisdom of Wisdom Teeth

Well they are not actually wisdom teeth but third molars.  Why do we have third molars? Evolutionarily they were there to give us an additional tooth at 15 years of age to aid in chewing due to the wear and tear on our other teeth.  Back then the food supply was a lot grittier containing dirt, sand and rocks.  After all, it was the caveman age.

Today our food is much less coarse and our teeth generally last longer.  The problem is for many individuals there is not enough room in their mouth for the third molars to come in.  This could be caused by genetics like dad is six feet tall and mom is four feet.  The combo can produce big teeth in a small jaw or vice versa.

Back in the day dentists would tell patients to keep their wisdom teeth because later in life they might be needed to support dental restorations.  Today with the advent of dental implants this is not necessary.

Third molars generally cause problems.  They are so far back in your mouth it is hard to keep them clean which can lead to decay or gum disease. Sometimes they only partially come in leading to more problems where an infection can develop between the partially exposed tooth and your jaw.

If an individual is on blood thinners and needs to have the wisdom tooth removed their cardiologist will generally refuse to take them off their medication. The extraction could cause real problems as the area will not stop bleeding after the procedure.

The morale here is that wisdom teeth in the vast majority of cases should be removed when you are around 16 years of age.   At this point your skeleton is still forming and not fully hardened.  Also the wisdom teeth have not developed legs or roots and are therefore smaller in size.

If the extractions are difficult it is easier to take a few days off when you are still in high school.  Leaving the wisdom teeth in can cause major problems if as they grow they decide to reside next to your mandibular nerve in your lower jaw or go into your upper sinus.
